Insurance Policy

Our services ranges from $85 – $199 for each visit depending on condition and medical need. Our therapeutic goals are to restore function, increase mobility, reduce pain, improve balance, and promote health & wellness. Results are not guaranteed as it will depend on your level of commitment on your home exercise program and consistency or continuity of care.

 

Currently we are “in-network” with Anthem Blue Cross PPO, Blue Shield of California PPO, and Medicare insurance plans. We are not providers for state funded Medicaid or Medi-Cal programs, or accept any personal injury liens.  If we are “In-network” with your insurance plan, we may need for you to sign an ABN form (advance beneficiary notice) for non-coverage services that you may be responsible for CASH out of pocket. For example, pre-season conditioning, return to golf, golf mobility programs, or cold laser treatments are considered maintenance and wellness services that most insurances don’t cover. If we are “Out of Network” with your insurance plan, you can pay out of pocket and submit a statement to your insurance for reimbursements.  Depending on your insurance plan, some companies may only offer 30% coverage or full reimbursements.   Otherwise, we also have specific cash package plans to help you return to your functional needs, or reach your athletic performance goals.  Please refer to cash pricing for details.

 

We advised that it is your responsibility to verify your insurance benefits for your deductible and co-insurance. Please as a reminder your DEDUCTIBLE must be met prior to your insurance coverage to take full affect. We will submit your insurance claim on your behalf as a courtesy. We may ask for a full payment for each visit if you do not know your insurance benefits. Please allow up to 6 weeks for claims processing and reimbursements. Once we receive payments from your insurance, we will refund you the difference. Any concerns or discrepancies with your insurance coverage please refer to your insurance company or call our billing department at (714) 283 – 3242.

 

We are in a business for health and wellness, and we do not discuss any financials at our facility. We accept cash or credit card for our services up front. If we are “in-network” with your insurance, then we will bill your charges on your behalf to your insurance company.  We also reserve the right to refuse services to anyone with or without a reason. Thank you for your understanding!

Payment

Cash, and major credit cards accepted for payment or PayPal or Venmo at DrJimmyHang@gmail.com (Venmo preferred with no incur service charges).  We also offer payments plans to allow some financial flexibility to our patients.

 

Cancellation Policy

If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you may be required to pay for the full cost of the session.
